Lisle Township now accepts Passport applications.

Applications for passports are taken:

Monday through Friday, from 8:30 until 4:00 p.m.

WE NOW TAKE PASSPORT PHOTOS AT THE TOWNSHIP OFFICE AT A COST OF $10.00

For those 16 years of age or older, passports will be valid for 10 years

For those under 16 passports will be valid for 5 years.

PASSPORT RENEWAL BY MAIL (Customer Submits) You may qualify to use this form only if: 1. The issue date of your last passport was less than 15 years ago; 2. You were 16 or older when it was issued; 3. Your name is the same or you can submit name-change documentation (marriage certificate court order); 4. You can submit this passport; and 5. EXPEDITED Service (Form DS-11 or DS -82)

Add $60.00 Per Application and 2-Way Overnight Delivery

Expedite fee does not include overnight delivery.

Customer must arrange for overnight delivery to ensure receipt within 2 weeks.

If international departure in less than 2 weeks, or passport is needed for foreign visas, application at a Regional Passport Agency may be advised. Appointment required.

You will need two checks.

Your passport fee payable to U.S. Department of State, (with applicant’s birth day in the memo portion of the check).

A second check for the Execution Fee made payable to Lisle Township.

To Apply You Will Need:

Proof of Citizenship (Birth Certificate, Naturalization Certificate, etc.)
ID (Drivers License, State ID, Military ID, etc.)
Two Passport Photos
Children under 14 years of age need both parents present or one parent appears and submits second parents written statement consenting to passport issuance for child or Statement of Consent Form DS-3053. (This statement must be notarized.)

· With Routine Service you will have your passport within 6 weeks.

· Add $60.00 per application for Expedited Service (receipt of passport in three weeks)

· Expedited Service fee and two-way Overnight Delivery stamps ($60.00 expedited fee plus two overnight delivery stamps at $13.65 each) for receipt of passport within two weeks.

If international departure is in less than 2 weeks, or passport is needed for foreign visas, application at the Chicago Passport Agency is advised. Call 312-341-6020 to arrange the required appointment.
