Lisle Township is proud to be a partner with Access DuPage. The Access DuPage program is a program to help eligible persons in DuPage County receive primary care medical services. Access DuPage is not an insurance program, nor is it a substitute for health insurance. But, if you are without health insurance and meet the eligibility criteria, Access DuPage and its participating physicians will try to provide you with a “medical home” where you can receive primary care services at a small cost until you become eligible for insurance.

Lisle Township residents may apply for Access DuPage at Supervisors office. Located at 4711 Indiana Ave in Lisle, one mile west of Il 53 and one block South of Ogden Ave. Appointments are required and are taken for Tuesdayas and Thursdays. Call 630-968-2087 ext 10 or 16 to make your appointment.
Documents needed during appointment:
- Photo Identification
- *Proof of Residency in Lisle Township (current utility bill, lease, tax statement)
- Social Security Card
- Income Documentation Over The Previous 90 Days
- Current Access DuPage Card (Renewal Appointments)
* If an applicant has no proof of residency documents, they must provide a signed, handwritten letter from the resident or landlord that the applicant is living with along with proof of residency of the letters author.
