LISLE TOWNSHIP ACCEPTS PASSPORT APPLICATIONS:

New Passport Fees - Effective July 13, 2010

Routine Service (Approximately 4 to 6 weeks)– Form DS-11

Passport Book

The U.S. Passport Book may be used for any type of international travel.

Passport Card

The U.S. Passport Card may only be used for re-entry into the United States at land borders and

sea ports-of-entry when traveling from Canada, Mexico, the Caribbean and Bermuda.

Passport cards may not be used for air travel.

IMPORTANT: When applying for both the Passport Book and the Passport Card only one execution fee will charged.

Beginning October 1, 2009 all passport applications are required to be send as

Priority Mail with Delivery Confirmation. The cost for this service is  $5.65

Expedited Service is available. Customers who choose expedited service must include an additional $60.00 per application. Expedited service is only available for passport books at this time. Expedited service will take 2 to 3 weeks. Those who need to travel in less than two weeks need to contact the Chicago Passport Agency (1-877-487-2778) to schedule an appointment at the Federal Building

Items needed to apply:

Proof of Citizenship (any one of the following):

Birth Certificate

Certificate of Naturalization

or previous Passport

Identification (any one of the following):

                        Drivers License

                        State Identification Card

                        Passport

Passport photos:

A recent clear likeness of the applicant. 2” by 2”, front view, full face, white or light color background. Passport photos are available at the Township Office for a fee of $10.00

Two checks – one for the passport payable to the U.S. Department of State

one for the execution & postage fees payable to Lisle Township.

 

All applicants must apply in person. Children under 14 years of age need both parents to be present or one parent & a notarized statement from the parent unable to be in attendance at the time of the application. (consent form DS-3053 is available on the Internet at www.travel.state.gov). A minor who is over 16 years of age can apply without a parent present if:

  1. They have a driver’s license
  2. Their parents are listed as the emergency contact
  3. The check to the U.S. Department of State is from their mother or father

 

Passport Renewal by Mail (applicant submits)

Complete this checklist to determine your eligibility to use form DS-82.

 

I can submit my most recent U.S. passport book and/or U.S. passport card with this application.

 

I was at least 16 years old when my most recent U.S. passport book and/or U.S. passport card was issued.

 

I was issued my most recent U.S. passport book and/or U.S. passport card less than 15 years ago.

 

I use the same name as on my most recent U.S. passport book and/or U.S. passport card.

-Or-

I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

 

When renewing by mail you are not required to appear before an acceptance agent or to pay the execution fee. The total cost to renew by mail is $75.00. Renewal by mail form DS-82. is available at Lisle Township, on line at www.travel.state.gov or at any of the nearly 9,400 acceptance facilities across the country.
