Overnight delivery add $16.25 to the Passport Fee

*The $25 Execution Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form

Valid For Travel by Sea, Air, or Land

New changes have been implemented in the passport program effective,

February 1, 2008.

·        Passport fees are now $75 with an execution fee of $25

·        All new applications (labeled 02-2008)

·        New Passport Card

·        Two- Parent Consent Age Change (from under 14 to under 16)

Travel requirements of the Western Hemisphere Travel Initiative (WHTI) have changed. Please go to www.travel.state.gov for details.

Validity:

·        Age 16 and over, the card is valid for 10 years.

·        Ages 15 and under, the card is valid for 5 years.

Size:

·       5 x 3 ½  in. (when unopened)

PASSPORT BOOK - ROUTINE PROCESSING

Age 16 and Over

·        Application form (DS-11) Do not sign the application

·         Two recent passport photos 2x2 in.

·        Previous Passport Book, Certified Birth Certificate (Must be certified by the state or county), Naturalization Certificate, etc. We cannot accept Hospital birth records.

Two Separate Fees

Fees: 

$ 75.00              Passport Fee – Check or Money Order

                           Made out to the U.S. Department of State

                           (Write date of birth on check memo line.)

$ 25.00              Execution Fee - Check, Money Order, Cash

                           (Made out to Lisle Township)

Teenagers 16 and 17 years of age are eligible to apply alone with the following requirements:

1.      Valid drivers license or state ID

2.      Personal checks made out by parent or guardian

3.      And all Documents listed above

·        Children under age 15 and under - both parents need to be present

unless one parent has a (DS-3053) form signed and notarized

·        Two recent passport photos 2x2

·        Certified Birth Certificate (Must be certified by the state or county), Naturalization Certificate, or Previous Passport, etc. We cannot accept Hospital birth records.

1.      For previous passports, children age 15 and under must submit both previous passport and birth certificate

2.      If child has had a passport and is misplaced, still must submit birth certificate and complete form (DS-64) for lost or stolen passport

PASSPORT BOOK – RENEWAL

Adults Only

·        Application renewals (DS-82) for adults only

·        Two recent passport photos 2x2

·        You can use DS-82 form if you answer Yes to the following:

1.      Can submit most recent Passport Book

2.      Issued less than 15 years ago

3.      At least 16 years of age when issued

4.      Presently using the same name on passport or have had name changed by marriage or court order and can submit proper documentation to reflect the change (Original certificates bearing official seal)

Air travel does not apply

Beginning February 1, 2008, U.S. citizens will be able to apply in advance for their passport cards. However, we anticipate that the passport card will be available sometime in the spring of 2008.

The passport card will provide a less expensive, smaller, and more convenient alternative way for those who travel frequently by land or by sea.

Validity:

·        Age 16 and over, the card is valid for 10 years.

·        Ages 15 and under, the card is valid for 5 years.
